Thank You For Submitting Your Intake Form

Prior to your appointment, it is important that we have a copy of your medical records and completed payment so that the approval process is quick and easy.

1. Payment: After submitting your intake form, we sent you an email with a link to pay for your evaluation online. Please complete your online payment at your earliest convenience. If you need assistance or would like to make payment by phone, please call us at 866-457-5559.

2. Medical Records: If you didn't upload records to your intake form, please email them directly to records@ohiomarijuanacard.com with your name in the subject line. You can also fax your records to us, but please be sure to do so at least 48 hours prior to your appointment. Once we've received your records, our team will review them to ensure they are sufficient to certify you as a medical marijuana patient.
